It is important for us to have your CAQH & NPPES/PECOS information on file so that we can manage your credentialing process with Medicare as well as our commercial insurance providers. Please follow the instructions below to retrieve your user ID and/or reset your password.

On the CAQH ProView homepage, locate and click on the "Login" or "Sign In" link to access the login page.
On the login page, you should see options for recovering your password or username. Typically, there are links or buttons labeled "Forgot Password?" or "Forgot Username?" Click on the appropriate option based on what you need to recover.
You will be asked to provide certain information to verify your identity. This may include your CAQH Provider ID, email address, National Provider Identifier (NPI), or other personal information. Follow the on-screen instructions and fill in the requested details accurately.
After providing the necessary information, CAQH will guide you on how to reset your password or retrieve your username. This typically involves sending an email to the address associated with your account.
Go to your email inbox and look for a message from CAQH. This email should contain instructions on how to reset your password or retrieve your username. Follow these instructions carefully.
If you are resetting your password, the email will likely contain a link or code that allows you to create a new password. Follow the provided instructions to set a new password.
If you were trying to retrieve your username, the email should include your username information.
Once you have successfully reset your password or retrieved your username, return to the CAQH ProView login page, enter your username, and the newly created or reset password.
You should now be able to access your CAQH account with your recovered username and password. Please fill out the appropriate user ID and password section in your onboarding document.
On the NPPES homepage, locate and click on the "Log In" or "NPPES Login" link to access the login page.
On the login page, you should see options for recovering your user ID or password. Look for links or buttons labeled "Forgot User ID?" or "Forgot Password?" Click on the appropriate option based on what you need to recover.
